ADVISEMENT BY TRAVEL AGENTS

 

Travel By Cannon warrants that each client has been provided with and advised to carefully review a copy of these Terms and Conditions and that the customer fully understands that he/she is subject to all said requirements and policies.

 

 

RESPONSIBILITY

 

Please read each statement – by giving your credit card or making payment for your travel arrangements, you agree, understand and accept the following terms and conditions:

 

I understand that Travel By Cannon are agents and independent contractors, which make arrangements with third party vendors for all FIT travel, cruises, tours, and package services.  Neither Travel By Cannon agents and/or independent contractors are agents of other parties providing travel-related services.  All travel documents are issued subject to the terms and conditions specified by such vendors, including airline contract of carriage.  I agree not to hold Travel By Cannon, nor any of its agents or employees, liable for; and by accepting these documents customer waives any claim against all such parties for; (a) any loss of or damage to property or injury to any person caused by reason of any defect, negligence, or other wrongful act or omission of, or any failure of performance of any kind by; any other transportation company; vendor or supplier; (b) any inconvenience, loss of enjoyment, mental distress or other similar matter; (c) any delayed departure, missed connections, substitutions of accommodations, terminations of service, or change in fares or rates; and (d) any cancellation or double booking of reservations or tickets beyond the control of Travel By Cannon.

 

Travel By Cannon does not accept responsibility for losses or additional expenses due to delays or changes, quarantine, sickness, strikes, war, weather, acts of God or other causes.  Such losses or expenses will have to be borne by the passengers. 

 

 

We strongly suggest that all clients protect themselves by purchasing Trip Cancellation or Trip Interruption insurance. 

 

1)    I choose to accept the vendor insurance and I agree to the terms of coverage as stated in the brochure.  OR

2)    I have chosen to accept Travel Guard for my travel insurance and I have received a copy of my policy with its coverages and terms.  If purchased within 14 days of trip deposit, pre-existing conditions will be covered.   OR

3)    If I decline the optional travel protection, I am required to sign a waiver provided by Travel By Cannon, stating such, prior to a deposit being made.

 


 

 

 

I have been advised to use a credit card as this may offer me the opportunity to dispute the charge should a vendor cease operating.  I understand that by paying for my travel arrangements with a check, cash, or debit card, I may risk losing these payments in the event of a travel supplier default, inclement weather or any other reason barring you from traveling as planned.

 

I understand that Travel By Cannon will not provide any reimbursement or travel credit in the event of any supplier default, inclement weather or any other reason barring you from traveling as planned.

 

I understand that if traveling internationally, I must have a valid passport and depending upon my destination and nationality, I may need to obtain one or more visas.   The passport must not expire less than six months before returning to the United States.

 

I have provided my Travel Counselor with my legal name as it appears on my government-issued I.D. or on my passport, if I am leaving the country.

 

It is my responsibility to check the vendor website for their respective terms and conditions and to share them and these terms and conditions with all traveling with me.

 

I will review my invoice and travel documents for accuracy upon receipt and understand that I must contact Travel By Cannon immediately if there are any discrepancies or if I have any questions.  I understand that discounted fares typically involve restrictions and that changing any aspect of my travel arrangements may result in the payment of additional money.

 

 

Retention of this confirmation for more than 24 hours constitutes acceptance of its accuracy and acceptance of our terms and conditions.

 

 

Consumer Disclosure Notice

 

PLEASE READ THIS NOTICE.  IT CONSTITUTES PART OF YOUR CONTRACT FOR TRAVEL RELATED SERVICES.  PLEASE CHECK YOUR DOCUMENTS WHEN YOU RECEIVE THEM.  CALL THE TRAVEL AGENCY IF YOU HAVE ANY QUESTIONS.

 

Incentive Connection Travel, Inc and Travel By Cannon, LLC (herein “Travel Agency”) is acting as a mere agent for Suppliers (identified on the accompanying documents/reservations) in selling travel related services, or in accepting reservations or bookings for services that are not directly supplied by this Travel Agency (such as Air and Ground Transportation, Hotel Accommodations, Cruises, etc.).  Travel Agency, therefore, shall not be responsible for breach of contract, failure to comply with any laws such as the Americans with Disabilities Act (ADA), or any intentional, negligent or careless actions or omissions on part of such suppliers, which result in any loss, damage, delay, inconvenience or injury to travelers or travelers’ companions or group members.  Unless the term "GUARANTEED" is specifically stated in writing on your tickets, invoice, or reservation itinerary, we do not guarantee any of such suppliers' rates, bookings, reservations, connections, scheduling, or handling of baggage or other personal effects.  Travel Agency shall not be responsible for any injuries, damages, or losses caused to any traveler in connection with terrorist activities, social or labor unrest, mechanical or construction failures or difficulties, diseases, local laws, climatic conditions, abnormal conditions or developments, or any other actions, omissions, or conditions outside the Travel Agent's control.  Traveler assumes complete and full responsibility for, and hereby releases the Agent from, any duty of checking and verifying and all passport, VISA, vaccination, or other entry requirements of each destination, and all safety and security conditions of such destinations, during the length of the proposed travel.  For information concerning possible dangers at international destinations, contact the Travel Advisory section of the U.S. State Department, (202) 647-5225, or access the State Department’s online travel advisory service (http://travel.state.gov/travel).  For medical information, call the U.S. Center for Disease Control (CDC), (404) 332 4559 or log on to www.cdc.gov/travel.  By embarking upon his/her travel, the traveler voluntarily assumes all risks involved in such travel, whether expected or unexpected.  Traveler is hereby warned of the above risks as well as possible Travel Industry bankruptcies and medical and climatic disruptions, and the possibility traveler may be unable to travel as scheduled because of personal reasons.  Traveler is advised to obtain appropriate insurance coverage against these risks; information is available through this Travel Agency regarding travel insurance.  Traveler's retention of tickets, reservations, or bookings after issuance shall constitute a consent to the above and an agreement on his/her part to convey the contents hereto to his/her travel companions or group members.
